PROGRAM
CODA South acts as a representative body for people who receive individualised
funding, and can provide case management of these funds on the individual's
behalf.
An Adult Lifestyle Package is designed to assist a person with a disability
to live in their own home (either family home or home of their choosing)
and access community life. Packages are usually applied for by an individual
and managed by a service organisation. Applications can be made through
DSQ, however CODA can assist with this process if you require it.
Whether it be community access or assistance in the home, CODA can tailor
a service delivery plan to meet your individual needs.

Other programs may require the development of individual programs, that
incorporate stimulating and meaningful day activities. CODA is committed
to developing and/or extending your identified goals. We work with individuals
and carers on a close basis and invite significant others to partake in
program planning, care plans and service reviews, usually on a 3-6 monthly
basis, or as needed.
